TMJ 1st Orthodontics
Orthodontists all over theworld diagnose cases by looking at Angle’s molar relationship and how the anterior teeth bite on hand held models. We have been doing this for 100 years – it is a rigid tradition. Angle classification is mainly a two dimensional, antero posterior analysis. However, once we start to focus more on TMJs, we start unveiling patient’s real problem in three dimensions.
Patients may be able to bite in centric occlusion where their teeth fir best while condyles are out of TMJ fossae, and many orthodontists are not paying enough attention to the joint, which is closely associated with occlusion in the mouth. It is problematic to diagnose 3 D problem cases with 2 D analysis. People may say, TMJ problem is for adults. On the contrary, if you look at recent studies by Nebbe and Major about prevalence of disc displacement among young pre orthodontic patients, they show that about 85% of girls and 60% of boys already have disc displacement.

A class II malocclusion can be manifested by different skeletal and dental configurations like maxillary proganthism, mandibular retrognathism or a combination of both. Most of the Class II cases are seen with mandibular retrognathism and if diagnosed correctly on time (during pubetral growth), it can be easily correctred skeletally. One of the successful appliances to correct such problem is the use of Twin Block appliance.

The lip bumper is a removable of fixed appliance that can be used for mandibular dental decompensation. This appliance shields the soft tissue from the dentition, allowing for spontaneous passive arch expansion. It works by altering the equilibrium between the cheeks, lips and tongue and by transmitting force from perioral muscles to the molars, where it is applied, hence it is particularly useful in patients who have very tight or tense buccal and labial musculature.

10th Arab Orthodontic Society Congress
The Arab Orthodontic conference is the prime scientific event of the Arab Orthodontic Society every year. This year the privilege of holding this prestigious event has been given to Emirates Orthodontic Society. In this conference the organizers would be focusing on the latest developments in the various aspects of the specialty. The straight wire low friction (SWLF) technique enjoys all the advantages of the traditional straight wire approach but eliminates one of its main failings which are static and dynamic friction. Although friction ensures occlusal stability and three dimensional control over the root in the last stages of treatment, it is equally true that it is also the principal obstacle to dental alignment and leveling, thereby complicating and prolonging treatments.

It is highly recommended that children see an orthodontist no later than age seven. There will be only a few orthodontic problems may be treated this early. But an early evaluation allows the orthodontist to determine if there would be future skeletal and dental problems.
